By submitting this form, I acknowledge the following:

• I agree to indemnify and hold harmless MTEF and MTEF sponsors and partners from claims, demands, and judgments arising at any time my child is participating in the MTEF Summer Tennis & Reading Program.

• I understand that MTEF includes educational activities as part of its program, and I give consent for my child to participate in these activities.

• I give my consent for my child to be administered first aid and to be treated by an emergency medical technician, paramedic, nurse, or physician.  Any follow up medical attention may be given at a local hospital and transportation to a Milwaukee hospital is authorized.

• I hereby give my consent for my child to be interviewed or photographed by the media or MTEF for purposes of website or newspaper publication.

• I understand that there is no transportation provided and it is my responsibility to drop off my child before his/her session and pick him/her up after the session.

Session Selection
Please select the date, time and site. Registration for both morning and afternoon sessions is permitted, but may be limited based on demand.

Applicants may apply for two or more sessions (MIMINUM 2 WEEKS). Space and/or time may be limited. Please rank your choices. (1 - first choice, 2 - second choice, etc.)
